Ladders/stairs/steps were not in good repair or were not slip resistant. The hand rail was loose.
Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair. TIGHTEN THE HAND RAIL. Correct By: 20-Jul-2018
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
One of the entry gates does not close on its own and the other does not latch on its own.

3701-31-04 (B)(6)(y) / Chemicals are properly stored/used
Critical Chemical containers were stored without labels identifying the contents. The smaller chlorine tanks were not labeled. No lid was on the Tri-chlor container. Liquid chemical was store on top of dry chemical.
Label and properly store all chemical containers to avoid a chemical reaction or unnecessary exposure. LABEL THE SMALLER CHLORINE TANKS. KEEP LIDS ON CONTAINERS OF CHEMICALS. DO NOT STORE DIFFERENT KINDS OF CHEMICAL ON TOP OF EACH OTHER. Correct By: 23-Jun-2018
