3701-31-04 (B)(1)(a) / Outlet covers shall be installed/secured/in compliance
Critical Repeat Outlet covers were not secured. Two screws are missing in each main drain cover.
Do not operate the pool when the outlet covers are not secured. Secure the covers prior to the pool being placed back into operation. INSTALL ALL 4 SCREWS IN THE MAIN DRAIN COVERS.
3701-31-04 (B)(6)(c) / Cross connection hazards shall be prevented.
Repeat Cross connection hazards were present. Hose bib connection in the equipment room is not back flow protected.
Remove all cross connection hazards from the pool area. INSTALL A VACUUM BREAKER ON THE HOSE BIB IN THE EQUIPMENT ROOM.
3701-31-04 (B)(6)(h) / Required meters and gauges maintained/functional
Required meters and gauges were not maintained or were not functional while the pool is in operation.
Maintain all meters and gauges in a properly functioning manner at all times while the pool is in operation. REPAIR FLOW METER. Correct By: 21-Jun-2023

The operating water level of the pool was not maintained.

Our records show that the main drain suction outlet covers (AquaStar WAV 12) were installed 8/1/2018. The covers have a 5 year life span and must be replaced before 8/1/2023. The cover has a flow rating of 274 gpm. When replacing the covers, ensure that the new covers' flow rating will not be exceeded.

Keep documentation of the ASSE 1013 backflow preventer’s annual inspection available at the pool facility for review by the health department.
