Ladders/stairs/steps were not in good repair or were not slip resistant. A step is missing on the ladder by the filter room.
Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair. REPAIR/REPLACE THE MISSING STEP ON THE LADDER BY THE FILTER ROOM. Correct By: 15-Nov-2017

Deck surfaces had standing water by the hand rail by the steps.

Critical Chemical containers were stored without labels identifying the contents.
Label and properly store all chemical containers to avoid a chemical reaction or unnecessary exposure. OBTAIN A CHLORINE SPECIFIC CONTAINER FOR THE LIQUID CHLORINE AND ENSURE THAT IT IS LABELED. DO NOT USE A CONTAINER THAT PREVIOUSLY HELD A DIFFERENT CHEMICAL. Correct By: 15-Nov-2017

12 ft. non-telescopic reach pole with shepherd's crook was not provided at the unguarded pool. The crook was not attached to the pole.
Provide a 12 ft. non-telescopic reach pole in the pool area at all times. ATTACH THE CROOK TO THE REACH POLE. Correct By: 21-Aug-2017
3701-31-04 (E)(2)(a)(ii) / A USCG Type IV personal flotation device with 30 -60 foot throw line provided
A USCG Type IV personal flotation device with 30 -60 foot throw line was not provided at the unguarded pool. The rope was only 18 ft.
Provide a proper floatation device at all times. ATTACH A 30-60 ROPE ONTO THE RING BUOY. Correct By: 21-Aug-2017

The unguarded pool was in use without the "Children must be supervised" and/or "Swimming alone is not recommended" signs posted.
Post the "Children must be supervised" and/or "Swimming Alone is Not Recommended" signs at all times the pool is available for use. Correct By: 15-Nov-2017

IT IS RECOMMENDED THAT YOU POST A SIGN IN THE POOL ROOM STATING THE LOCATION OF THE FIST AID KIT. IT MUST BE MADE AVAILABLE TO PATRONS.

OPERATOR TESTING RECORDS WERE NOT FOUND. FOLLOW THE TESTING FREQUENCY AND PROCEDURE PER RULES (SEE HANDOUT).
