Corrected During Inspection Backflow prevention devices were not maintained to function properly. The hose bib on the outside of the equipment room is not protected with an ASSE 1011 backflow preventer.
Have the backflow prevention device evaluated or replaced so that backflow is prevented. INSTALL AN ASSE 1011 BACKFLOW PREVENTER ON THE HOSE BIB OUTSIDE OF THE EQUIPMENT ROOM.

Have the backflow prevention device evaluated or replaced so that backflow is prevented. WATER IS FILLED AT A FILL SPOUT LOCATED IN THE EQUIPMENT ROOM. REPORTEDLY, THIS FILL SPOUT IS PLUGGED AND NO LONGER USED. AN DIRECT CONNECTION FROM THE MAIN WATER LINE HAS BEEN MADE TO THE POOL WITH A HOSE CONNECTION. AN ASSE 1015 BACKFLOW PREVENTER IS IN BETWEEN THE MAIN WATER LINE AND THE POOL. EITHER DISCONNECT THIS CONNECTION AND REMOVE THE HOSE AND HOSE BIB OR INSTALL AN ASSE 1013 BACKFLOW PREVENTER TO REPLACE THE ASSE 1015 BACKFLOW PREVENTER.
3701-31-04 (B)(6)(c)(ii) / Backwash lines and lines from drainage sumps shall have an air gap and remain open to freely drain.
