3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Repeat Public swimming pool was constructed, installed, or substantially altered without obtaining approval from the Ohio Department of Health (ODH).
Contact ODH to obtain approval for any construction/alterations before operating pool. ODH HAS NOT NOTIFIED LCPH OF THE APPROVAL FOR THE EQUIPMENT REPLACEMENT. REPORTEDLY, THE ERN HAS BEEN SUBMITTED TO ODH.
3701-31-04 (B)(1)(a) / Outlet covers shall be installed/secured/in compliance
Critical Repeat Outlet covers were not secured.
Do not operate the pool when the outlet covers are not secured. Secure the covers prior to the pool being placed back into operation. REPLACE THE MAIN DRAIN COVERS AND INSTALL NEW COVERS PROPERLY FOLLOWING MANUFACTURER'S INSTRUCTIONS. Correct By: 22-Oct-2021
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
pH was outside the range of 7.2 to 7.8.
Treat the pool water so that the pH is within the acceptable range. ADJUST pH AFTER REFILLING Correct By: 30-Jul-2021
3701-31-04 (D)(05) / Cyanuric acid level shall not exceed 70 ppm.
Repeat Cyanuric acid level exceeded 70 ppm
Drain the pool at least halfway and fill with clean water. Monitor the amount of cyanuric acid being used and adjust accordingly. REDUCE CYA. Correct By: 29-Jul-2021
