Critical Chemical containers were stored without labels identifying the contents. Bag of chemicals was directly on the floor.
Label and properly store all chemical containers to avoid a chemical reaction or unnecessary exposure. STORE BAGS OF CHEMICALS OFF OF THE FLOOR. ADDITIONAL STORAGE RACKS MAY BE NEEDED. Correct By: 09-May-2019
3701-31-04 (D)(7)(d) / Flow or pressure switch with a shut off valve shall be provided and tested monthly.
Critical Flow or pressure switch was not tested monthly.
Provide records indicating that the unit is being tested monthly. TEST AND RECORD THE ACC FLOW SWITCH MONTHLY. SEND ME A COPY OF THE RECORD SHEET INDICATING THE TEST RESULTS WITH 7 DAYS. Correct By: 09-May-2019
