Do not operate the pool when the disinfectant residuals are less than what is required. Ensure the chlorine disinfectant residual is at 1 ppm or above prior to placing the pool back into operation. THIS IS A CRITICAL OPERATIONAL ITEM AND POSES AN IMMINENT HEALTH HAZARD. OPERATOR CLOSED THE POOL AND ADJUSTED THE FREE CHLORINE. AFTER 1 HOUR THE CHLORINE WAS 2 PPM. Correct By: 09-Jun-2022

Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve. INCREASE FLOW RATE TO AT LEAST 408 GPM.

Keep the chemicals’ Safety Data Sheets (SDS) in the equipment room and have Personal Protective Equipment (PPE) such as chemical gloves and goggles available for handling chemicals.

The Certificate of Compliance for the main drain cover(s) is/are available in the equipment room. Our records show that the main drain suction outlet covers (Waterway 640-472xV) were installed 5/1/2016. The covers have a 7 year life span and must be replaced before 5/1/2023. This drain cover has a maximum flow rating of 356 gpm. When replacing the covers, ensure that the new covers' flow rating will not be exceeded. 408 gpm
