| Equipment requiring ODH plan approval has been replaced since the last inventory report dated 8-10-16. The Challenger CH11-N1-2FE pump was changed to a Pentair Whisperflo WF-28 pump. The Flex Flo A19020A-6T disinfectant feeder was replaced with the Flex Flo A1N1OA-7T disinfectant feeder. Submit an Equipment Replacement Notification Form to the Ohio Department of Health for the replacement of the pump and for the disinfectant feeder if the new feeder has a different capacity than the old feeder. An ERN form was supplied. |
