3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Repeat Equipment, at a public swimming pool, was changed/altered without obtaining approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN). Chemical feeder pump was replaced last year without ODH approval.
Obtain approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN) before operating pool. SUBMIT THE ERN FOR THE REPLACEMENT OF THE DISINFECTION FEEDER PUMP. Correct By: 01-Jul-2022
3701-31-04 (B)(1)(a) / Outlet covers shall be installed/secured/in compliance
Critical Outlet covers were not secured. Screw missing in the main drain cover.
Do not operate the pool when the outlet covers are not secured. Secure the covers prior to the pool being placed back into operation. REPLACE THE MISSING SCREW IN THE MAIN DRAIN COVER. Correct By: 29-Jun-2022
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Perimeter barrier had gaps greater than 4 inches.
Repair the perimeter barrier so that there are not any gaps greater than 4 inches anywhere along the barrier. ELIMINATE GAP UNDER FENCE IF GREATER THAN 4 INCHES. Correct By: 13-Jul-2022

The Certificate of Compliance for the main drain cover(s) is/are available in the lifeguard room. Our records show that the main drain suction outlet covers (AquaStar WAV12) were installed on 6-9-19. The covers has a 5 year life span and must be replaced before 6-9-24.
