The Certificate of Compliance is not available for the main drain cover. One was supplied to operator at the time of inspection. Keep a copy at the facility for future review. Our records show that the main drain suction outlet covers (Paramount SDX 159-2212) were installed 9/10/2018. The covers have a 5 year life span and must be replaced before 9/10/2023.
