Repeat Backflow prevention devices were not maintained to function properly
Have the backflow prevention device evaluated or replaced so that backflow is prevented. INSTALL BACK FLOE PREVENTERS ON ALL HOSE BIBS.
3701-31-04 (B)(6)(c)(ii) / Backwash lines and lines from drainage sumps have an air gap and are open to freely drain
Repeat Backwash lines and lines from drainage sumps did not have an air gap prior to discharge
Open the lines that discharge into the backwash or drainage sumps so that there is an air gap and the lines can drain freely. INSTALL AN AIR GAP ON THE FILTER BACKWASH LINE.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve. INCREASE FLOW TO AT LEAST 408 GPM Correct By: 15-May-2019

Repeat Ladders/stairs/steps were not in good repair or were not slip resistant.
Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair. TIGHTEN LADDERS
3701-31-04 (B)(6)(y) / Chemicals are properly stored/used
Critical Repeat Chemical containers were stored without labels identifying the contents.
Label and properly store all chemical containers to avoid a chemical reaction or unnecessary exposure. REMOVE BAG OF CHEMICAL FROM THE FLOOR.
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
Repeat pH was outside the range of 7.2 to 7.8.
Treat the pool water so that the pH is within the acceptable range. Correct By: 18-Aug-2018
