3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Public swimming pool was constructed, installed, or substantially altered without obtaining approval from the Ohio Department of Health (ODH).
Contact ODH to obtain approval for any construction/alterations before operating pool. RESUBMIT THE ERN TO ODH FOR THE REPLACEMENT OF THE DISINFECTION FEEDER, THE SVRS, AND CIRCULATION PUMP.
3701-31-04 (B)(1)(a) / Outlet covers shall be installed/secured/in compliance
Critical Outlet covers were not installed.
Do not operate the pool when the outlet covers are not installed. REINSTALL THE SKIMMER EQUALIZER OUTLET PLUGS. Correct By: 06-Jun-2022
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
pH was outside the range of 7.2 to 7.8.
Treat the pool water so that the pH is within the acceptable range. ADJUST pH Correct By: 06-Jun-2022

Keep the chemicals’ Safety Data Sheets (SDS) in the equipment room and have Personal Protective Equipment (PPE) such as chemical gloves and goggles available for handling chemicals.

The Certificate of Compliance for the main drain cover(s) is/are available in the equipment room. Our records show that the main drain suction outlet cover (Waterway 640-472xV) was installed 4/1/2018. The cover has a 7 year life span and must be replaced before 4/1/2025

The operator was observed performing onsite chemical tests appropriately and is aware of the acceptable parameters and necessary corrective actions.
