3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Equipment, at a public swimming pool, was changed/altered without obtaining approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN) -Observed the Challenger 120073A pump was replaced with a Harris 72525. SUBMIT AN EQUIPMENT REPLACEMENT NOTIFICATION TO ODH.

3701-31-04 (B)(6)(h) / Required meters and gauges maintained/functional
Required meters and gauges were not maintained or were not functional while the pool is in operation. -Observed the flow meter to be at 100gpm, which is below the minimum flow rate to achieve turn over in 8 hours, and the lowest range for the flow meter. The flow meter did not appear to be working properly. CLEAN THE FLOW METER AND DETERMINE IF THE GAUGE IS FUNCTIOING PROPERLY.
Maintain all meters and gauges in a properly funtioning manner at all times while the pool is in operation. Correct By: 05-Jun-2023

Either the flow meter is not operating properly or the system is not achieving a full turnover in 8 hours. Ensure the flow meter is operating properly. If the flow meter is accurately reading below 108 gallons per minute, have the system evaluated and adjusted to achieve a proper flow rate.

The Certificate of Compliance is not available for the main drain cover. One was supplied to operator at the time of inspection. Keep a copy at the facility for future review. Ensure that the installation date is also available.
