Have the backflow prevention device evaluated or replaced so that backflow is prevented. HAVE THE ASSE 1013 BACKFLOW PREVENTER THAT IS ON THE FILL LINE INSPECTED AND RETAIN THE INSPECTION REPORT FOR THE HEALTH DEPARTMENT. Correct By: 15-Jan-2018

Critical The drain covers are missing two screws on each cover and one screw is rusty.
PLACE THE SCREWS IN THE DRAIN COVERS AS DIRECTED BY THE MANUFACTURER AND USE ONLY THE MANUFACTURER'S SCREWS WHICH ARE STAINLESS STEEL. Correct By: 15-Jan-2018

"WATER TESTING AND RECORD KEEPING", "CRITICAL OPERATIONAL ITEMS", AND CDC FECAL ACCIDENT RESPONSE HANDOUTS WERE SUPPLIED TO THE OPERATOR AND REVIEWED TO ENSURE THAT THE OPERATOR IS KNOWLEDGEABLE OF THE REQUIRED CHEMISTRY LEVELS, CRITICAL ITEMS WHERE THE POOL MUST CLOSE, AND HOW TO RESPOND TO A FECAL ACCIDENT. THE OPERATOR SEEMED AWARE OF THESE ITEMS.
