[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Public swimming pool was constructed, installed, or substantially altered without obtaining approval from the Ohio Department of Health (ODH). The pump was changed. New pump is a Pentair WF-4. The previous pump was a Hayward SP3207X10.
Contact ODH to obtain approval for any construction/alterations before operating pool.
3701-31-04 (B)(6) / Equipment is maintained per manufacturers specifications.
Equipment was not maintained per manufacturers specifications. Heater is leaking water.
Repair heater leak.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (B)(6)(q) / Depth markers/deck warning signs are maintained
4 ft Depth marker is coming off the deck.
Re-attach the 4ft depth marker on the deck.
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
The SW back door is damaged and does not self-close.
Repair the back door so it self-closes and latches
3701-31-04 (B)(6)(y) / Chemicals are properly stored/used
Critical Chemical containers were left open and liquid chemical were stored over dry chemicals on the shelves in the equipment room.
Properly store all chemical containers to avoid a chemical reaction or unnecessary exposure. Keep containers closed. Rearrange chemicals so liquids are not above dry on shelves.
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
pH was outside the range of 7.2 to 7.8.
Treat the pool water so that the pH is within the acceptable range.
