[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (B)(4)(a)(i) / Disinfection residuals are tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Disinfection residuals were not tested daily at pool side and/or at the required intervals for pools/automatic chemical controllers/special features. -Observed the free chlorine is not tested and recorded every day. On the days that it is recorded, it's not recorded at least every 4 hours that the pool is open. If the pool is closed, make sure that's documented.
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use.
3701-31-04 (B)(4)(a)(ii) / Combined chlorine is tested daily at pool side prior to use
Combined chlorine was not tested daily at pool side prior to use. -Observed the comobined chlorine is not tested every day, and when it is tested, it's not every 4 hours that the pool is open. -Test the combine chlorine prior to bathers entering the pool and at least every 4 hours the pool is open. If the pool is closed, make sure that's documented.

pH was not tested daily at pool side and/or at required intervals for pools/automatic chemical controllers/special features. -Observed the pH is not tested daily, and when it is recorded, it's not at least every 4 hours tha the pool is open. -Test the pH before bathers enter the pool and at least every 4 hours the pool is open. If the pool is closed, make sure that's documented.

3701-31-04 (B)(4)(a)(iv) / Water temperatures are recorded daily
Water temperatures were not being recorded daily. -Observed the water temperature is not being recorded every day. -Record the water temperature at least daily that the pool is open. If the pool is closed, make sure that's documented.
Take and record the water temperature at least once/day.
3701-31-04 (B)(4)(c) / SVRS/APSOS/ACC test results/ service records are maintained
SVRS/APSOS/ACC test results/ service records were not maintained. -Observed the SVRS is not being recorded that it's tested at least monthly. -Test the SVRS system at least montly and record the results.
The test results/service records for these devices must be kept on site for review. If the device has not been recently serviced, have the device tested immediately and provide documentation of the test results.
3701-31-04 (B)(6)(p) / Deck surfaces are free of standing water or other hazards.
Deck surfaces had standing water or other hazards. -Observed an area of broken concrete on the deck near the chairs. -Observed standing water on the deck near the steps into the pool. -Repair areas of the deck where standing water is occuring. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval.

3701-31-04 (D)(8)(c) / Test kit reagents fresh/ properly stored.
Test kit reagents were not fresh or stored properly. -Observed almost all the reagents in the test kit were expired. -Replace expired reagents with fresh reagents and store them properly.
