[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Equipment, at a public swimming pool, was changed/altered without obtaining approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN). Obseved the pump was replaced without submitting an ERN to ODH. PROMPTLY SUBMIT AN ERN TO ODH.

3701-31-04 (B)(6)(c)(ii) / Backwash lines and lines from drainage sumps shall have an air gap and remain open to freely drain.
Backwash lines and lines from drainage sumps did not have an air gap prior to discharge. Observed an air break, not an air grap for the backwash lines. ADJUST THE PLUMBING TO ENSURE THERE IS A PROPER AIR GAP IN THE DISCHARGE LINE.

3701-31-04 (B)(6)(t) / Equipment/chemical/storage areas are free of hazards/accessible only to authorized personnel.
Critical Equipment/chemical/storage areas were not secured. Observed the fenced in area with the pump and a large container of hypochlorite was not locked, and anyone in the pool area can access this chemical or the equipment in the fence. ENSURE THIS FENCED IN AREA WITH EQUIPMENT AND CHEMICALS IS ONLY ACCESSIBLE TO AUTHORIZED PERSONNEL.
These storage areas must be kept inaccessible to everyone but authorized personnel. Secure the areas.
3701-31-04 (B)(6)(y) / Chemicals are properly stored/used
Critical Chemical containers were stored without labels identifying the contents. -Observed a large container of sodium hypochlorite without a label stored in the fenced in area near the pump. PROMPTLY LABEL ALL CHEMICALS.
Label and properly store all chemical containers to avoid a chemical reaction or unnecessary exposure.
3701-31-04 (D)(8)(c) / Test kit reagents fresh/ properly stored.
Test kit reagents were not fresh or stored properly. Observed expired reagents in the test kit. PROMPTLY REPLACE ANY EXPIRED REAGENTS.
Obtain fresh reagents and store them properly.
