3701-31-04 (B)(1)(a) / Outlet covers shall be installed/secured/in compliance
Critical Repeat Outlet covers were not secured. Screws missing in both covers.
Do not operate the pool when the outlet covers are not secured. Secure the covers prior to the pool being placed back into operation. REPLACE THE MISSING SCREWS IN BOTH MAIN DRAIN COVERS.
3701-31-04 (D)(7)(d) / Flow or pressure switch with a shut off valve shall be provided and tested monthly.
Critical Repeat Flow or pressure switch was not tested monthly
Provide records indicating that the unit is being tested monthly. RECORD THE ACC FLOW SWITCH TEST RESULTS ON THE LOG SHEET.
