| This inspection verifies the the Safety Vacuum Release System equipment replacement is consistent with plans submitted and approved in accordance with OAC 3701-31-05. ERN for the installation of a SVRS was approved by the Ohio Department of Health on 11-1-17 and is on file. Equipment requiring ODH plan approval has been replaced since the last inventory report dated 8-24-16. The Pentair Challenger CHII-N1-11/2 1.5 horsepower pump was replaced with a Hayward SP2610X15 1.5 horsepower pump. No Equipment Replacement Notification has been submitted and approved by the Ohio Department of Health. |
