3701-31-04 (A)(1) / Public swimming pool shall be licensed
Swimming pool is operating without a 2022 pool license.
Call Local Health Department and make arrangements to pay for the 2022 pool license and late fee. Also provided contact information to include address and phone of new management company and the name and phone of local individual that will be observing and maintaining pool daily conditions.
3701-31-04 (B)(6)(c)(i) / Backflow prevention devices shall be maintained.
Backflow prevention devices were not maintained to function properly
Have the backflow prevention device evaluated or replaced so that backflow is prevented.
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
Repeat pH was outside the range of 7.2 to 7.8
Treat the pool water so that the pH is within the acceptable range.
3701-31-04 (D)(7)(d) / Flow or pressure switch with a shut off valve shall be provided and tested monthly.
Critical Flow or pressure switch for a shut off valve was not provided or the unit was not tested monthly
Have the ACC unit serviced or replaced with a unit that has a flow or pressure switch with an automatic shut off valve. Provide records indicating that the unit is being tested monthly.
3701-31-04 (E)(2)(e) / Working emergency telephone/call box provided.
Repeat Emergency device (telephone) was not working.
Ensure that the emergency device is functioning properly and rings to the emergency response authorities.
3701-31-04 (E)(3)(a) / "WARNING, NO LIFEGUARD", sign posted
The unguarded pool was in use without the "WARNING, NO LIFEGUARD" sign posted.
Post the WARNING, NO LIFEGUARD sign in a conspicuos location at all times the pool is available for use.

Disinfection tank requires a label on the outside to identify its contents.
Make sure to provide information to ODH when making equipment changes to the circulation system.
