Repeat Equipment, at a public swimming pool, was changed/altered without obtaining approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN).
It appears that an ERN has been submitted to ODH, but approval has not be given yet.

Repeat Ladders/stairs/steps were not in good repair or were not slip resistant.
Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair. REPLACE THE MISSING TREADS ON THE LADDER RUNGS.

Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair. TIGHTEN THE HANDRAIL TO THE STEPS.
3701-31-04(D)(6)(a)(1) / Combined chlorine shall not exceed 1.0 ppm.
Combined chlorine was greater than 1.0 ppm.
Treat the pool so that the combined chlorine is less than 1 ppm. REDUCE COMBINE CHLORINE LEVEL. INCREASE VENTILATION WHILE SHOCKING THE POOL.
