Backflow prevention devices were not maintained to function properly
Have the backflow prevention device evaluated or replaced so that backflow is prevented. Backflow prevention device should be inspected annually and should be a model ASSE 1013.
3701-31-04 (B)(6)(k) / Outlet drain covers are secure/meet ANSI/ASME A112.19.8-2007 requirements.
Critical Observed the east drain cover missing a screw.
Drain covers are required to be secured with all 4 screws.
3701-31-04 (E)(2)(a)(ii) / A USCG Type IV personal flotation device with 30 -60 foot throw line provided
A USCG Type IV personal flotation device with 30 -60 foot throw line was not provided at the unguarded pool. -Observed two USCG Type IV personal floatation devices in pool area. One floatation device ha a 30-60 foot rope securely attatched and the 2nd floatation device has no 30-60 rope securly attached.
Attach rope to 2nd floatation device. Only one flotation device is required with this indoor pool.
