3701-31-04 (B)(6)(h) / Required meters and gauges maintained/functional
Repeat Required meters and gauges were not maintained or were not functional while the pool is in operation.
Maintain all meters and gauges in a properly functioning manner at all times while the pool is in operation. REPAIR FLOW METER. Correct By: 02-Mar-2024
3701-31-04 (D)(7)(d) / Flow or pressure switch with a shut off valve shall be provided and tested monthly.
Critical Repeat Flow or pressure switch for a shut off valve was not provided or the unit was not tested monthly
Have the ACC unit serviced or replaced with a unit that has a flow or pressure switch with an automatic shut off valve. Provide records indicating that the unit is being tested monthly. TEST AND RECORD ON THE LOG SHEET THE RESULTS OF THE ACC FLOW SWITCH FUNCTION.
3701-31-04 (D)(8)(c) / Test kit reagents fresh/ properly stored.
Test kit reagents were not fresh or stored properly. Reagents 003, and the alkalinity 007, 008, 009 are expired.
Obtain fresh reagents and store them properly. DISCARD EXPIRED REAGENTS AND USE FRESH REAGENTS. 003, 007, 008, 009. Correct By: 02-Mar-2024

Equipment requiring ODH plan approval has been replaced since the last inventory report dated 2-15-22 without submitting an ERN to ODH. The Disinfection Chemical feeder pump LMI Milton Roy A151-92T was replaced with a LMI Milton Roy 8D851-833SI.

Safety Data Sheets (SDS) for the sodium hypochlorite are kept in the storage room. I could not find the SDS for the other chemicals. Personal Protective Equipment (PPE), chemical gloves and goggles, should be available for handling chemicals.

The Certificate of Compliance for the main drain covers is available in the office.
Our records show that the main drain suction outlet covers (Waterway 640-472xV) were installed 4/9/2019. The covers have a 7 year life span and must be replaced before 4/9/2026. These drain covers have a maximum flow rating of 356 gpm. When replacing the covers, ensure that the new covers' flow rating will not be exceeded. 450 gpm.
