Critical Outlet covers were not compliant covers. One skimmer equalizer is missing a plug.
Do not operate the pool without compliant outlet covers. Install compliant outlet covers prior to placing the pool back into operation. PLUG THE EQUALIZER OUTLETS AT THE POOL WALL. Correct By: 09-Jun-2023

Critical Outlet covers were not secured. A screw is missing in the main drain.
Do not operate the pool when the outlet covers are not secured. Secure the covers prior to the pool being placed back into operation. INSTALL ALL 4 SCREWS IN THE MAIN DRAIN COVER.
3701-31-04 (B)(1)(b) / SVRS device(s) shall be functional.
Critical The SVRS device that is being used as a secondary means to prevent entrapment/evisceration was not functional at the time of the inspection.
Do not operate the pool when the SVRS is not functioning. Ensure the SVRS is operating properly prior to placing the pool back into operation. REPAIR THE SVRS. Correct By: 05-Jun-2023

All equipment must be maintained in accordance with the manufacturers specifications. ADJUST THE SKIMMER ON THE NORTH SIDE OF THE POOL SO IT DRAWS WATER. Correct By: 09-Jun-2023
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Repeat Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve. INCREASE FLOW RATE TO BETWEEN 76 AND 98 GPM.

Critical Chemical containers were stored without labels identifying the contents.

The unguarded pool was in use without the "Swimming alone is not recommended" signs posted.
Post the "Swimming Alone is Not Recommended" signs at all times the pool is available for use. POST REQUIRED SIGN. Correct By: 09-Jun-2023

CDC does not recommend chlorine levels above 10 ppm.

Keep documentation of the ASSE 1013 backflow preventer’s annual inspection available at the pool facility for review by the health department.

Our records show that the main drain suction outlet cover (Waterway 640-472xV) was installed 7/1/2018. The cover has a 7 year life span and must be replaced before 7/1/2025.

The little test kit in the equipment room has expired reagents. The kit should be discarded.

If the pump is replaced submit an ERN to ODH if the pump is not identical.

If the ACC is not operational, the chlorine and pH must be tested every 4 hours.
