The test results/service records for these devices must be kept on site for review. If the device has not been recently serviced, have the device tested immediately and provide documentation of the test results. TEST AND RECORD SVRS MONTHLY AND BEFORE REOPENING.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Repeat Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve. INCREASE FLOW RATE TO 76-98 GPM.

Repeat The unguarded pool was in use without the "Children must be supervised" and/or "Swimming alone is not recommended" signs posted.
Post the "Children must be supervised" and/or "Swimming Alone is Not Recommended" signs at all times the pool is available for use. POST REQUIRED SIGNS.
