3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Public swimming pool was constructed, installed, or substantially altered without obtaining approval from the Ohio Department of Health (ODH). A new fence was installed without approval.
Contact ODH to obtain approval for any construction/alterations before operating pool. CONTACT THE OHIO DEPARTMENT OF HEALTH 614-466-1390 FOR APPROVAL FOR THE NEW FENCE. Correct By: 09-Jun-2022
3701-31-04 (B)(6)(c) / Cross connection hazards shall be prevented.
Repeat Cross connection hazards were present
Remove all cross connection hazards from the pool area.
3701-31-04 (B)(6)(c)(i) / Backflow prevention devices shall be maintained.
(REPEAT) Backflow prevention devices were not maintained to function properly
Have the backflow preventer replaced so that backflow is prevented. INSTALL THE CORRECT BACKFLOW PREVENTER, AN ASSE 1013, ON THE FILL LINE THAT FILLS THE POOL.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Repeat Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve. INCREASE FLOW RATE TO AT LEAST 76 GPM.
3701-31-04 (B)(6)(q) / Depth markers/deck warning signs are maintained
Depth markers/deck warning signs were not maintained in a clearly legible/visible manner. Markings are more than 25 ft apart, Marking of 1/2 is used, 4 ft marking is worn off.
Replace the depth markers/deck warning signs so that bathers are able to clearly see the depth of the water and that no diving is permitted where clearly indicated. REDO THE DEPTH MARKINGS. ADD ONE MARKING SO THEY ARE NO MORE THAN 25 FT APART, USE 6 IN IN PLACE OF 1/2 FT, AND REPAINT THE 4 FT MARKING. Correct By: 09-Jun-2022
3701-31-04 (E)(2)(b)(i) / 12 ft. non-telescopic reach pole with Sheppard's crook & a USCG Type IV personal flotation device with 30 - 60 foot throw line.
12 ft. non-telescopic reach pole with Shepherd's crook and/or a USCG Type IV personal flotation device with 30 - 60 foot throw line was not provided at the guarded pool.
Provide the non-telescopic reach pole and personal floatation device at the pool at all times. PLACE IN THE POOL AREA A REACH POLE WITH SHEPHERD'S CROOK. PLACE IN THE POOL AREA A RING BUOY WITH A 30 FT ROPE. Correct By: 04-Jun-2022

Safety Data Sheets (SDS) are kept in the equipment room and Personal Protective Equipment (PPE), chemical gloves and goggles, should be available for handling chemicals.

The Certificate of Compliance for the main drain cover(s) is/are available in the equipment room. Our records show that the main drain suction outlet cover (Waterway 640-472xV) was installed 7/1/2018. The cover has a 7 year life span and must be replaced before 7/1/2025.
