[?] A summary of the violations found during the inspection are listed below.

3701-31-04 ( D )(6)(a) / ORP reading shall be 650 millivolts or greater.
ORP reading was below 650 mv. Automatic Chemical Controller reading 641 mV at this time.
Have the ACC unit inspected to determined why the ORP is low. Clean the probes and adjust setting so ACC reads 650 mV or have ACC serviced by pool company.
3701-31-04 (B)(4)(c) / SVRS/APSOS/ACC test results/ service records are maintained
Safety Vacuum Release System (SVRS) test results/ service records were not maintained.
The test results/service records for these devices must be kept on site for review. If the device has not been recently serviced, have the device tested immediately and provide documentation of the test results. Test and record the SVRS monthly.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Required turnover rates were not met. Flow meter reading at 70 - required flow rate must be at least 76 (per volume of pool/turnover rate).
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
