3701-31-04 (B)(1)(a) / Outlet covers shall be installed/secured/in compliance
Critical Repeat Outlets are not operating however they are not covered properly or removed or plugged. ANSI/ASME A112.19.8-2007 cover required.
Replace covers, or remove covers , fill the equalizer line and plug
3701-31-04 (B)(4)(c) / SVRS/APSOS/ACC test results/ service records are maintained
Repeat SVRS/APSOS/ACC test results/ service records were not maintained.
The test results/service records for these devices must be kept on site for review. If the device has not been recently serviced, have the device tested immediately and provide documentation of the test results TEST AND RECORD AT LEAST ONCE A MONTH .
