Critical Outlet covers were not secured. Two screws are missing in each of the main drain covers.
Secure the covers. PLACE SCREWS IN THE MAIN DRAIN COVERS SO THEY HAVE ALL 4 SCREWS IN PLACE. Correct By: 09-Sep-2022
3701-31-04 (B)(6) / Equipment is maintained per manufacturers specifications.
Repeat Equipment was not maintained per manufacturers specifications. One basket in the skimmer is too small that the size of the skimmer opening. Two skimmer have barrel weirs that are too small.
All equipment must be maintained in accordance with the manufacturers specifications. SIZE THE BASKET AND WEIRS IN THE SKIMMERS TO THE SKIMMER UNIT.
3701-31-04 (B)(6)(c) / Cross connection hazards shall be prevented.
Repeat Cross connection hazards were present. The hose bib in the equipment room is not back flow protected.
Remove all cross connection hazards from the pool area. INSTALL A VACUUM BREAKER ON THE HOSE BIB IN THE EQUIPMENT ROOM.
