Public swimming pool was constructed, installed, or substantially altered without obtaining approval from the Ohio Department of Health (ODH).
Contact ODH to obtain approval for any construction/alterations before operating pool. SUBMIT AN EQUIPMENT REPLACEMENT NOTIFICATION FORM TO THE OHIO DEPARTMENT OF HEALTH FOR THE REPLACEMENT OF THE DISINFECTION CHEMICAL FEED PUMP. Comments
Keep the chemicals’ Safety Data Sheets (MSDS) in the equipment room and have Personal Protective Equipment (PPE) such as chemical gloves and goggles available for handling chemicals.

Our records show that the main drain suction outlet cover (AquaStar 1216) was installed 7/1/2018. The cover has a 7 year life span and must be replaced before 7/1/2025.
The Certificate of Compliance was not available for the drain cover. One was supplied to operator at the time of inspection. Keep a copy at the facility for future review.
