Gates or doors were not self-closing and self-latching. Gate latch was locked, making the unable to latch.
Repair gates or doors in order to be self-closing and lockable. KEEP THE GATE ON THE WOMEN'S REST-ROOM SIDE EITHER UNLOCKED SO THAT THE GATE WILL LATCH AFTER OPENING OR KEEP THE GATE LOCKED SO IT CANNOT BE OPENED. Correct By: 24-Jun-2022

HAVE THE BACKFLOW PREVENTER INSPECTED ANNUALLY.

CHECK WITH THE 9-1-1 OPERATOR TO ENSURE THEY HAVE THE CORRECT ADDRESS.

When using CYA (cyanuric acid) the alkalinity level is calculated by subtracting 1/3 the CYA level from the alkalinity reading. EXAMPLE: alkalinity test reading = 70 ppm, CYA = 60 ppm. Alkalinity is 70 ppm – (60/3) = 50 ppm

Safety Data Sheets (SDS) were not found in the equipment room, but Personal Protective Equipment (PPE), chemical gloves and goggles, are available for handling chemicals.

The Certificate of Compliance for the main drain cover(s) is/are available in the equipment room. Our records show that the main drain suction outlet covers (Lawson MLD SG 1212) were installed 9-3-18. The covers have a 10 year life span and must be replaced before 9-3-28.

The operator was observed performing onsite chemical tests appropriately and is aware of the acceptable parameters and necessary corrective actions.
