3701-31-04 (B)(1)(a) / Outlet covers shall be installed/secured/in compliance
Critical Repeat Outlet covers were not compliant covers.
Do not operate the pool without compliant outlet covers. Install compliant outlet covers prior to placing the pool back into operation. PLUG THE SKIMMER EQUALIZER OUTLETS AT THE POOL WALL.
3701-31-04 (B)(1)(c) / Minimum disinfection residual shall be maintained as required.
Critical Chlorine disinfectant residual was less than 1 ppm for the pool
Do not operate the pool when the disinfectant residuals are less than what is required. Ensure the chlorine disinfectant residual is at 1 ppm or above prior to placing the pool back into operation. OPERATOR ADJUSTED THE ORP AT THE TIME OF INSPECTION. CHECK FREE CHLORINE LEVEL BEFORE OPENING. IF CHLORINE LEVEL IS NOT AT LEAST 1 PPM DO NOT OPEN POOL. Correct By: 14-Jul-2022
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve. INCREASE FLOW RATE TO AT LEAST 142 GPM. Correct By: 16-Jul-2022

Critical Outlet covers did not meet ANSI/ASME A112.19.8-2007 standards. Grating is broken.
Replace damaged outlet covers with covers that meet the ANSI/ASME A112.19.8-2007 standard. REPLACE THE DRAIN COVER. POOL MUST CLOSE UNTIL REPLACED. Correct By: 15-Jul-2022
