Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.

3701-31-04 (E)(2)(a)(ii) Repeat A USCG Type IV personal flotation device with 30 -60 foot throw line was not provided at the unguarded pool. Provide a proper floatation device at all times.

3701-31-04(D)(6)(a)(2) Critical Pool free chlorine disinfectant residual was below 1 ppm. Do not operate the pool until the chlorine disinfectant residual is at 1 ppm or above.

Chlorine was a little low. Life saving floatation device needs the 30 foot rope included with it. The flow rate was under the design standard for the volume of water in your pool. Please look into whether this can be increased to at least 90 gpm.
