3701-31-04 (B)(4)(c)
SVRS/APSOS/ACC test results/ service records were not maintained. Operator demonstrated knowledge of how to test automatic chemical controller safety shut off interlock mechanism; however, rules require that documentation of such testing (on a monthly basis) be maintained. A check-box with notation for pump shut off testing during each backwash procedure will be created on the daily log sheet.
The test results/service records for these devices must be kept on site for review. If the device has not been recently serviced, have the device tested immediately and provide documentation of the test results.

Equipment identification labels were not affixed to the filters. Operator has the information in a seperate file off site.
Replace the missing equipment labels or have the proper equipment information on site for each piece of equipment.

Repeat pH was outside the range of 7.2 to 7.8.
Treat the pool water so that the pH is within the acceptable range.

12 ft. non-telescopic reach pole with Shepherd's crook and/or a USCG Type IV personal flotation device with 30 - 60 foot throw line was not provided at the guarded pool.
Provide the non-telescopic reach pole and personal floatation device at the pool at all times.

Sign indicating the location of the emergency device (telephone) was not provided and emergency device was not visible in the pool area.
