3701-31-04 (B)(4)(a)(ii)
Combined chlorine was not tested daily at pool side prior to use.
Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the combined chlorine must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

Required turnover rates were not met. According to the flow meter the rate is 300 gpm which is well below the required 722. Maybe it is just a problem with the flow meter?
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (E)(2)(b)(i)
12 ft. non-telescopic reach pole with Shepherd's crook and/or a USCG Type IV personal flotation device with 30 - 60 foot throw line was not provided at the guarded pool. Just need the 30 ft. of rope for the ring buoy.
Provide the non-telescopic reach pole and personal floatation device at the pool at all times.
3701-31-04(D)(6)(a)(2)
Critical Pool free chlorine disinfectant residual was below 1 ppm. Measured at 0.4ppm.
Do not operate the pool until the chlorine disinfectant residual is at 1 ppm or above.
