3701-31-04 (B)(6) Equipment was not maintained per manufacturers specifications. The basketball hoop was observed loose and in disrepair. Repair per manufacturer's specification. All equipment must be maintained in accordance with the manufacturers specifications.

3701-31-04 (B)(6)(p) Repeat Deck surfaces had standing water or other hazards. To the right side of the main pool entry, observed the deck surface in disrepair creating the potential for a trip hazard. Repair areas of the deck where a trip hazard is present.
