3701-31-04 (B)(6) Equipment was not maintained per manufacturers specifications. Observed the Shepards Crook missing a bolt. Repair. All equipment must be maintained in accordance with the manufacturers specifications.

3701-31-04 (B)(6)(p) Repeat Deck surfaces had standing water or other hazards. Observed chipped concrete in multiple locations surrounding the lap pool. Repair. Repair areas of the deck where standing water is occuring. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval.

Ensure that all main drains are secure and up to date on certification and not expired. Provide proof that these requirements have been met. If expired, the drain covers must be replaced immediately. Inspection conducted with Shane Snapp.
