| 3701-31-04 (B)(6)(o) Repeat Ladders/stairs/steps were not in good repair or were not slip resistant. Observed ladder next to lifeguard post by the rock-wall able to be pulled completely out of its slot. Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair. |
| 3701-31-04 (B)(6)(p) Deck surfaces had standing water or other hazards. Observed chipped concrete throughout the deck. Repair areas of the deck where standing water is occuring. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval. |
