3701-31-04 (E)(2)(a)(ii) At the time of inspection, the USCG Trype IV flotation device was observed without a 30-60 ft throw line. To ensure proper function, include a 30-60 foot throw line to the flotation device or obtain a new flotation device that includes a 30-60 ft throw line.

- Ensure the drain line in the pump room has an air gap. - At the time of inspection, it appears the existing pump was replaced with a pump from the spa, resulting in an increase in horsepower. If a pump is replaced, and it is not like for like, an equipment notification form needs to be submitted to the Ohio Department of Health for approval. Complete and send to ODH an equipment replacement notification form. - Inspection was conducted by not only Jason Dreier but also Carson Richardson.
