Critical Public swimming pool was in operation while an imminent health hazard existed. Observed entrapment hazard at rope bridge on special feature/ play feature.
PIC (person in charge) is placing/stationing a lifeguard on at the rope bridge to continuously monitor this area at all times. PIC understands that if this condition (lifeguard at proper station) is not met the entire activity pool will close until the imminent health hazard is eliminated.

Repeat Special features were not maintained in good repair or in accordance with the manufacturer specifications. 1.The netting on the special feature/play feature has a substantial gap present which creates an entrapment hazard underneath. The netting shall be fastened down or tightened to prevent a potential entrapment hazard. Correction: Repair/maintain all the special features in good repair and in accordance with the manufacturer's specifications. If necessary, replace any broken components present.

Comments
Main focus on this follow-up inspection was to ensure entrapment hazard was fixed or eliminated before the opening day of the pool.

Entrapment hazard is still existing under/with the rope bridge of the special feature/play feature in the activity pool. The two sided white netting that goes to the pool bottom, are both loose and can be pulled up to a height that may allow a person to slide under the netting structure and be trapped. The following agreement was made to allow the pool to open until this issue is resolved.
-> PIC (person in charge) is placing/stationing a lifeguard on at the rope bridge to continuously monitor this area at all times.
-> PIC understands that if the above condition (lifeguard at proper station) is not met the entire activity pool will close until the health hazard is eliminated

The above issue is in violation of the Ohio's Recreational Water Code:
3701-31-04 (B)(1) - Public swimming pool shall operate free of an imminent health hazard. This is a critical violation

After reviewing the past files and reports on this specific rope netting feature, Miami County Public Health has determined that this feature must be installed back to the original state to prevent any hazards. This means full netting must be secured to all netting attachment areas on poles or pillars. As approved previously by the Ohio Department of Health.

The PIC is requesting Ohio Department of Health to perform a consult inspection with Miami County Public Health, to assist in reviewing this netting and issue. MCPH will contact ODH for consultation.
