3701-31-04 (E)(2)(e) At the time of inspection, the emergency telephone was observed in disrepair. To ensure authorities are contacted during an emergency, the emergency telephone shall be maintained in good repair. Repair or replace the emergency telephone.

- Please provide lifeguard certifications prior to opening. - At the time of inspection, all the safety equipment, excluding emergency telephone, was present and in good repair. Also, the chemical readings were within normal limits. - Reinspection will be conducted prior to opening on May 26 to check the repairs or replacement of the emergency telephone.
