3701-31-04 (B)(3) According to the records provided, on multiple days the chlorine residual was observed below the required 1.0 ppm, however, no corrective actions were observed. To ensure proper procedures are being met, the staff shall be trained/knowledgeable in what to do if the free chlorine drops below 1.0 ppm. Ensure the staff is knowledgeable and documents corrective actions if the free chlorine is below 1.0 ppm.

3701-31-04 (B)(6)(h) Repeat At the time of inspection, the flow meter was observed nonfunctional. To ensure proper flow and thus the proper turnover rate, the flow meter shall be in good repair. Repair or replace.
