Special features were not maintained in good repair or in accordance with the manufacturer specifications. A portion of the base to the special feature display was observed loose and in disrepair. Repair per manufacturer's specification to eliminate the potential entrapment hazard. Between the section of the water slide, observed water leaking. Ensure any gaps are eliminated and leaking sections are tightly secured. The fabric barriers on either side of the special feature play slide was observed not securely fastened at either end. Repair the fabric barrier to ensure an entrapment hazard is eliminated
Repair/maintain all the special features in good repair and in accordance with the manufacturer's specifications. If necessary, replace broken components.

At the time of inspection, the fabric barrier to the slide for the special play feature was observed inadequately sealed at either end of the slide. To eliminate a potential entrapment hazard, the pool operator stated the manufacturer will be contacted to install a more tightly fastened and secured barrier on both ends.
Until the issue is fixed, the pool operator agreed that a lifeguard would be staffed specifically at this location to ensure patron safety.

Also at the base of the special feature play structure (south side), the metal paneling was observed coming loose, creating a gap and potential entrapment hazard. At the time of inspection, the pool operator was able to contact maintenance to have this issue addressed. Until the issue has been fixed, a lifeguard must also staff this area of the play feature to ensure patron safety.

The netting barrier along the path leading to the pump room was observed completely removed from the stationary wooden pole for access to the pump room by staff. The netting barrier on the opposite side to the unauthorized area below the waterslides was also completely removed and unfastened.
ODH is being contacted for their opinion on this issue. Once the Health Department receives word from the State of Ohio Department of Health, the facility will be contacted and made aware of what needs to occur to address the issue.

The ACC was voluntarily switched out at the time of inspection. The reason for this change was concern about the ACC not properly alarming when tested. The PIC stated the ACC has had maintenance performed on it on 3 separate occasions and has been having issues with alarming.

Based on today's inspection, a re-inspection has been scheduled to occur on or after 7/28. Once the special play feature (fabric barrier on either ends of the slide and the loose metal paneling) has been fixed, contact the Health Department for a follow-up inspection to ensure compliance.
