Repeat Deck surfaces had standing water or other hazards. Near the depth marker at the deep end to the right hand side of the rightmost diving board, a severe crack in the cement was present creating a potential trip and/or toe hazard.
Repair areas of the deck where a trip and/or toe hazard is present. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval.
Comments
According to the Person-in-Charge, the lap pool's pump motor stopped functioning about 3 weeks ago. The motor to activity pool's special play feature was swapped out to continue powering the lap pool.
This pump motor is not meant to power the lap pool, thus Management stated the originally approved motor will be re-installed.

Once installed, provide proof in an email to csuper@miamicountyhealth.net that this change has been made back to its original.

Also, ensure the facility's weekly pool operations and incident report sheet are being cross-referenced to match the State document (HEA 5219) to ensure all required test parameters are being documented and recorded.
