3701-31-04 (B)(1)
Critical Repeat At the time of inspection, the circulation system was observed not functioning and the automatic chemical controller was unable to be tested for low or no flow. During the inspection, the pool was closed ("Danger Pool Closed Due to imminent health hazard" sign posted). Continue to cease operation while there is an imminent health hazard present. Prior to reopening, have the disinfection system properly running and be able to test the automatic chemical controller. Contact the heath department for an inspection.

3701-31-04 (B)(1)(d)
Critical Repeat At the time of inspection, the automatic chemical controller was observed reading an ORP of 805 with a free chlorine level of 4.2 and the disinfection system was not functioning. Cease operation of the pool until the circulation system is functioning properly. Contact the health department for an inspection.

3701-31-04 (B)(1)(e)
Critical Repeat At the time of inspection, the automatic chemical controller was unable to be tested for low or no flow. Cease operation of the pool until the automatic chemical controller is able to be function properly and tested for low or no flow.

3701-31-04 (B)(4)
Repeat At the time of inspection, did not observe any records being maintained. During operation document and keep for 2 years water quality readings, testing of equipment, etc.

3701-31-04 (B)(4)(a)(i)
Repeat At the time of inspection, the free chlorine was observed not being tested or recorded. While in use for a pool with an automatic chemical controller, test the free chlorine prior to opening and every 12 hours.

3701-31-04 (B)(4)(a)(ii)
Repeat At the time of inspection, the combined chlorine was observed not being tested or recorded. To ensure the combined chlorine is below the required 1.0 ppm, test the combined chlorine prior to opening and every 12 hours.

3701-31-04 (B)(4)(a)(iii)
Repeat At the time of inspection, the pH was observed not being tested or recorded at the necessary intervals. To ensure the pH is within normal limits, the pH shall be tested and recorded prior to opening and every 12 hours.

3701-31-04 (B)(4)(a)(iv)
Repeat At the time of inspection, the water temperature was observed not being taken or recorded. Take and record the water temperature at least once a day.

3701-31-04 (B)(4)(a)(vi)
Repeat At the time of inspection, records were not provided that the alkalinity was being tested weekly. Ensure the alkalinity is tested once a week.

3701-31-04 (B)(4)(c)
Repeat At the time of inspection, records were not found indicating the automatic chemical controller no or low flow switch was being tested monthly. Have the ACC low or no flow switch tested and documented monthly.

3701-31-04 (C)(3)
Repeat At the time of inspection, the total alkalinity was observed at 40 ppm. To ensure the water is properly balanced, the alkalinity shall be at or above 60 ppm. Increase the alkalinity.

3701-31-04 (D)(02)
Critical Repeat At the time of inspection, the pool was observed being disinfected by placing pucks into the skimmer baskets (hand feeding and not continuously disinfecting). Cease hand feeding and have the disinfection system repaired or replaced so that the pool is continuously disinfected. Cease operation of the pool until this done. Once completed, have the pool inspected by the health department prior to opening.

3701-31-04 (D)(7)(d)
Critical Repeat The low or no flow on the automatic chemical controller was unable to be tested. Cease operation of the pool until the low or no flow on the ACC can be tested. Also, ensure it is tested and documented monthly.

3701-31-04 (E)(2)(e)
Repeat At the time of inspection, the emergency telephone was observed not working. To ensure authorities are able to be contacted, an emergency telephone shall be available and functional. Repair or replace the emergency telephone.

Comments
- At the time of inspection, the pool was already closed with the "Warning this pool is Closed" sign posted. The gate was also locked. With the office PIC, discussed keeping the pool closed and ensuring the pool is not hand fed. The PIC did state repairs are needed and they are in the process of getting that completed. For replacement of any piece of equipment, contact the Ohio Department of Health Swimming Pool Program staff. When equipment is repaired or replaced, water quality readings are within normal limits, and safety equipment is present and in good repair, contact the health department for an inspection.
