3701-31-04 (B)(1)(a)
Critical Repeat At the time of inspection, the equalizer line covers were observed not properly secured to the adjacent wall. Secure the equalizer line to the adjacent wall.

3701-31-04 (B)(6)(o)
Repeat The ladders and rails were observed without escutcheon plates. To prevent the pooling of water, install escutcheon plates to ladders and rails.

3701-31-04 (D)(8)(c)
Repeat The DPD test kit was observed without fresh reagents. Discard the old reagents and obtain new ones.

3701-31-04 (E)(2)(e)
Repeat At the time of inspection, the emergency telephone was nonfunctional. Repair or replace the emergency telephone.

3701-31-04 (E)(3)(b)
Repeat At the time of inspection, "Swimming alone is not recommended" and "Children must be supervised" signs were not observed. Obtain signs with the aforementioned statements.

The following violation(s) have been corrected since the last inspection.
3701-31-04 (B)(1)(l)/Electrical hazards shall not exist.
The underwater light was observed not connected to a GFCI breaker or outlet. Connect the underwater light to a GFCI breaker.
3701-31-04 (D)(7)(d)/Flow or pressure switch with a shut off valve shall be provided and tested monthly.
At the time of inspection, the flow switch on the automatic chemical controller was unable to be tested. Contact manufacturer on how to test ACC and test. Ensure testing is performed monthly and documented.
Comments
- Reinspection was conducted to check the critical violations (testing of the automatic chemical controller, securing of the equalizer line covers and installing a GFCI breaker on the underwater light). At the time of inspection, the testing of the automatic chemical controller was performed by the PIC. Also, temporarily the underwater light was removed from power until a new GFCI breaker is able to be obtained. Securing of the equalizer lines was still needed to be completed. A reinspection will be conducted on 6/4 to check the equalizer line covers along with the other items.
