At the time of inspection, tiles located where the spa was located was observed in disrepair.
Repair or replace the deck tiles to eliminate tripping and cutting hazards.

Repeat According to the records provided, the chlorine residual was observed not being tested prior to opening and every 4 hours.
To ensure the free chlorine is at or above 1.0 ppm, the chlorine residual shall be tested prior to opening and every 4 hours. Test the free chlorine prior to opening and every 4 hours.

Repeat According to the records provided, the combined chlorine was observed not being tested prior to opening and every 4 hours.
Prior to opening and every 4 hours, to ensure the combined chlorine is at or below 1.0 ppm, the combined chlorine shall be tested. Test the combine chlorine prior to opening and every 4 hours.

- To prevent hazards, on the ladder in the 5 ft end, place an end cap on the bottom.

- At the time of inspection, all the safety equipment was present and in good repair. Also, the water quality readings were observed within normal limits.
