3701-31-04 (B)(4)(a)(i)
Repeat Disinfection residuals were not tested daily at pool side and/or at the required intervals for pools/automatic chemical controllers/special features.
Disinfection residuals are not being tested prior to open. The pool opens at 6:00 am, and records indicate that the testing is around 8:00 am-9:00 am. It is also not being tested every 4 hours. Ensure that the disinfection residuals are tested prior to open and every 4 hours.

Repeat Combined chlorine was not tested daily at pool side prior to use.
Combined chlorine is not being tested prior to open. The pool opens at 6:00 am, and records indicate that the testing is around 8:00-9:00 am. It is also not being tested every 4 hours. Ensure that the chlorine is tested prior to open and every 4 hours after.

Ladder is not in good repair
Repair the ladder at the deeper end of the pool. Upon inspection observed one of the covers for the end of the ladder to be missing which could cause someone to get cut. Ensure the cover is replaced.

Repeat Deck surfaces not smooth
Repair areas of the deck where tiles are elevated and broken causing a trip hazard.
3701-31-04 (C)(2)
pH was outside the range of 7.2 to 7.8.
Upon inspection, observed the pH to be around 8.0. Ensure the pool is treated to be in range of 7.2 to 7.8.
3701-31-04 (E)(2)(d)
First aid equipment was not sufficient
At time of inspection, first aid kit was missing basic items. Ensure the first aid kit has band aids, gloves, antiseptic wipes and antibacterial ointment.
Comments
At time of inspection, observed the records not being properly maintained. The pool opens at 6:00 am, but the water is not tested at open nor every 4 hours.

At time of inspection, pH was not within range of 7.2-7.8, it was read at 8.0, ensure pH is within range.

Observed the deck tiles in need of repair, as well as the ladder in the deeper end of the pool. The ladder does not have a cover on one of the ends, ensure it is replaced to prevent patrons getting cut.

One of the flotation devices has a tear in it, dispose of it and keep the one that is in good repair.
