3701-31-04 (B)(1)(a)
Critical At the time of inspection, the operator was unable to provide documentation the main drain cover was within its life span. Provide documentation or replace the main drain cover to ensure its within its life span. At the time of inspection, the PIC stated the main drain cover will probably be replaced.

3701-31-04 (B)(1)(b)
Critical At the time of inspection, the SVRS (Vac-Alert) was unable to be tested (not functioning). Upon making the PIC aware of this, the operator voluntarily ceased operation of the spa. When SVRS is functioning properly and able to be tested, contact the health department for a reinspection.Continue to cease operation of the spa until proper function is observed.

3701-31-04 (B)(6)(a)
At the time of inspection,tiles lining the spa were observed in disrepair. To prevent hazards, repair or replace the pool lining.

3701-31-04 (C)(7)
At the time of inspection, the spa water temperature was observed at 105.3 F. Reduce the water temperature to at or below 104 F.

3701-31-04 (E)(3)(e)
At the time of inspection, the caution spa sign was observed without the statement "Unsupervised use by children is prohibited". Include the aforementioned statement in the caution spa sign.

3701-31-04(D)(6)(a)(1)
At the time of inspection, the combined chlorine was observed at 2.8 ppm. To ensure the water is properly disinfected, reduce the combined chlorine to at or below 1.0 ppm.

Comments
- Ensure the spa water is replaced at least every 30 days and documented.

- Because the Vac-Alert was unable to be tested, see violation, the spa was voluntarily closed. After the inspection, the operator sent an email stating a new SVRS will be installed. Contact the Ohio Department of Health, Environmental Engineering, to see if Equipment replacement form is required (614-466-1390). Once installed, a reinspection will be conducted.
