3701-31-04 ( D )(6)(a) ORP reading was below 650 mv. Observed large pool at 645 mv. Have the ACC unit inspected to determined why the ORP is low.

3701-31-04 (B)(6) Equipment was not maintained per manufacturers specifications. Observed large slide leaking in two different locations. Locations were pointed out to operator for repair. All equipment must be maintained in accordance with the manufacturers specifications.
